Free Diabetes Prevention Program open to the Lebanon community

Join your health home for free classes that will embolden you to make healthful choices that fuel your well-being by learning ways to prevent diabetes!

About the program

Union Community Care is offering you the knowledge and tools to make healthful choices that fuel your well-being by learning ways to prevent diabetes! The free classes, which are offered in English and Spanish, kick off on Wednesday, October 4, 2023. The year-long classes pair you with a trained lifestyle coach, walk you through the CDC approved curriculum, and provide you with group support.

Who can join 

The classes are open to any Union Community Care patient or community member who has had an A1C test in the past 90 days that was between 5.7% to 6.4%.

Class dates

Orientation is on Wednesday, October 4, at either 10 a.m. or 5 p.m. Classes run from Wednesday, October 11, 2023 to Wednesday, October 9, 2024.

Class location

The classes are held at our Lebanon Medical location at 960 Church Street, Lebanon.
