Coordinated Care
The purpose of your Care Team is to know and understand your health history, focus on your care, and strengthen your well-being. This means that your whole team is caring for you, answering your questions, and connecting you to resources.
Accessible Services
Patients can call Union Community Care during open hours by calling one of our locations. After open hours, patients can talk to the on-call provider who can help with any urgent medical need. Patients can also connect with their Care Team at any time through the patient portal.
