Our Mobile Care Team is breaking down barriers to care in areas with limited access to health services and increasing the wellness of our community.

What is the Mobile Care Team?

It’s our first mobile medical unit! Think of it as an exam room on wheels. It’s a van staffed with one Union Community Care medical provider and one medical assistant to mobilize accessible and affordable healthcare that will meet people where they are and remove obstacles that keep people from getting and staying healthy.

What services does the Mobile Care Team provide?

The Mobile Care Team is able to provide a wide range of primary and preventive care, including Well-Child Checks, immunizations, school and sports physicals, sick care, and more.

Where will I see the Mobile Care Team?

The Mobile Care Team’s Spring and Summer routes will include school events that support students, staff, and families in preparing for the upcoming school year through School-Based Health Care, various community events, and an assortment of locations in mostly rural Lancaster County for our Plain Healthy Program.

Who can use the Mobile Care Team?

The Mobile Care Team is for anyone who needs in-the-moment health services or help finding a health home. New or established patients and community members with or without insurance can seamlessly receive care through the Mobile Care Team and at any other Union Community Care location.

How can the Mobile Care Team support me?

The Mobile Care Team is mapping out routes that meet you where you are remove obstacles that keep you from getting and staying healthy, such as available transportation, childcare challenges, and non-traditional work schedules. We offer many different payment plans and programs that keep our services affordable – no one will ever be turned away for care if they can’t pay.
